Cardiologists at Manipal Hospital Successfully Performs Rare Ballon Assisted BASILICA with TAVI Procedure, Saves 70-year-old woman from High-risk Open-Heart Surgery

Minimally invasive heart valve replacement performed successfully at Manipal Hospitals Kharadi Pune

Cardiologist at Manipal Hospitals Kharadi Pune successfully performed lifesaving Ballon- Assisted BASILICA with Transcatheter Aortic Valve Implantation (TAVI) on a 70-year-old woman with severe aortic stenosis and multiple comorbidities.

A patient with a history of obesity, hypertension, hypothyroidism, COPD and OSA suffered dyspnea and chest pain due to severe aortic valve stenosis. She had high estimated risk of morbidity and death for an open heart valve replacement surgery. Considering overall risk and recovery time the family preferred a minimally invasive procedure rather than an open heart surgical valve replacement, and so the doctors suggested transcatheter aortic valve implantation (TAVI).

Speaking about the case, Dr. Tanmai Yermal Jain, Consultant Interventional Cardiologist, Manipal Hospitals, Kharadi, said, “Symptomatic Severe Aortic Stenosis in a 70 years old patient indicated Aortic valve replacement surgery. This was a high-risk case due to patient's age, comorbidities and increased chances of coronary artery obstruction. Patients cardiovascular anatomy added a further layer of complexity: narrow sinuses of Valsalva, calcified valve leaflets, and very low-lying coronary arteries, all of which increased the chances of coronary artery obstruction during TAVI. We decided to perform Balloon-Assisted BASILICA technique to reduce this risk followed by TAVI. This procedure uses electric guidewires with balloon support to intentionally split the damaged valve leaflet, allowing blood to flow freely and preventing the valves from obstructing the coronary artery flow and then new Aortic valve was implanted at damaged aortic valve. This complicated procedure was performed through leg without any external stitches. Patient was mobilised on next day of procedure and safely discharged on 4th day.”
